Health Forms
All new students, kindergarteners and 5th grade students require physical examinations and immunization updates prior to the start of the school year. All students waiving immunizations need to have an immunization waiver form on file. Please check with your family physician to make sure your child is up-to-date.
Medication Forms
Any students taking medication during school hours need the proper forms filled out and on file in the school office. Prescription medications require a doctor’s signature, non-prescription medications do not. All medications are kept in the school office and are administered by staff. Students are not allowed to self administer or keep medications in their backpacks or desks. All medications kept at school need to be in their original packaging.

Athletics
Students in grades 4-8th grade are allowed to participate in extracurricular athletics at St. Paul. Our athletic programs are part of the Badgerland Lutheran Athletic Conference (BLAC). We offer volleyball, cross country, soccer, basketball, cheer, softball, track and VEX Robotics. Other extracurriculars include: band, choir, forensics, LEGO club and running club.
